Ask your employer if they participate in a matching Gift or workplace-giving
program.

Obtain a matching gift form from your employer (either from Human resources or
Payroll).

Complete the donor section of the form. Mail form with cheque, made payable to

| Your gift to SKCT must be a donation that can be called matching donation or other gift. |
